What is revive weight loss program?

Revive weight loss program is a hybrid, high level support weight loss program that combines individual Nutrition Counseling (1:1 sessions initial + follow ups) with a group accountability component. This structure allows you to receive personalize care while benefiting from community support, education and accountability and the best part? Insurance may cover the individual sessions!

What insurances is accepted?

I accept Blue cross blue shield, Aetna, Amerihealth, United Health Care

What If you don’t accept my insurance?

If your insurance is not accepted, a superbill can be provided for potential reimbursement! But this depends on your individual plan.

What is covered in 1-on-1 sessions (insurance eligible)

Individual sessions are personalized and will include: A personalized nutrition plan, meal timing and structure, nutrition support for many medical conditions (insulin resistance, PCOS, prediabetes, diabetes, Cholesterol and thyroid concerns, postpartum, GLP-1 Medications), Behavior change and goal setting with progress monitoring. These sessions are conducted privately on a HIPAA compliant platform and documented according to clinical and insurance guidelines.

What is included in the group portion of this program?

This portion is non-medical care (which is why insurance won’t cover!) and held in a private facebook group. it’s designed for support and accountability. The group component includes: Accountability and motivation, Weekly goals and wins, Meal Planning and weekly meal ideas, Grocery store guidance, exercise inspo, general Q&A related lifestyle habits. Weekly meal ideas will be provided including vegan options if during orthodox fasts.Prefer Privacy? Anonymous posting options are available, only the dietitian can see who posted!

Who is this program for?

This program is for any adult who would like to lose weight, build healthier habits, improve lab markers (Blood sugar, cholesterol, etc) I will follow a orthodox fasting schedule and will provide vegan options when needed, however, you do not need to be orthodox to join! There will be both options!

What is the refund & cancelation policy?

Due to the structure of this program, no refunds are offered & No cancelations once enrolled except in cases of medical necessity. However, I’m SOOOO confident that you will LOVE this program!

What is the cost?

Group Portion of program fee: Early Bird pricing- $150/month x 3 months. 1 on 1 and Follows ups should be covered 100% by insurance. If it's not-- $150 per 1 hour session, $100 per 30 min follow up. (a super bill will be provided if i'm not in network with your insurance for possibly reimbursement!) FSA/HSA accepted.
